Executive Summary

Puerto Rico’s hospital market remains dominated by general short-term hospitals, while bed capacity is unevenly distributed across regions and ownership groups. Although total reported bed capacity declined modestly from 2011 through 2024, the distribution of those beds provides important context for evaluating access, market concentration, and competitive position.

1. Puerto Rico Hospitals by Facility Type

General short-term hospitals remained the dominant hospital category throughout the period. In the most recent year, Puerto Rico reported approximately 48 general short-term hospitals submitting Medicare cost reports. The next largest category was Psychiatric, with approximately 4 hospitals. This chart provides market structure context before reviewing utilization and financial indicators. As stated by CMS, “Medicare-certified institutional providers are required to submit an annual cost report to a Medicare Administrative Contractor (MAC).”, hence there is a reasonable expectation that most hospitals certified in Puerto Rico are accounted for in these statistics.

2. Puerto Rico Hospitals Beds by Region

Total reported beds across regions declined by 6.1% over the period. Regional bed capacity remains uneven across Puerto Rico. In the most recent year, the region with the largest reported bed count was SAN JUAN, with approximately 2,414 beds. Total reported beds by ownership / control group declined by 6.1% over the period. Among the three main groups, Proprietary hospitals reported the largest bed count in the most recent year, at approximately 3,900 beds.
